Registration Process for COVID19 Antigen Test for CMU Students
Testing is held on 2nd January 2022 and registration is open till 1st January 2022.
Venue : 08.30 – 11.30 at the 30th Anniversary Building, Faculty of Sciences, Chiang Mai University
13.30 – 16.30 at Research and Technology Transfer Building, Faculty of Engineering, Chiang Mai University
***Choose a timeslot for a COVID19 antigen test first and then pay for the fee before visiting the link for booking an appointment
- Pay for the fee (350 Baht) via a mobile banking application before booking an appointment for a COVID19 antigen test. Please transfer money to the following account AMS Clinical Service Center Account Number 968-0000-990
- Register for the COVID19 antigen test at http://cmu.to/5FgOP. Registration Password is 93084. The student must select either a morning timeslot from 08.30 - 11.30 or an afternoon timeslot from 13.30 – 16.30 only. (50 students per hour.)
- Please complete the registration information and attach the money transfer slip to confirm your COVID19 antigen test appointment.
- After completing registration and submitting the transfer slip, the student will receive a QR code via SMS or E-mail in order for them to show the staff at the testing venue. If you do not receive a QR Code please contact 064-3747513 during working hours.
The COVID19 antigen test result will be sent to the student within 2 hours via SMS and via the Mohpromt Application.
For appointment cancellations, the student needs to inform the AMS Clinical Service Center 7 days prior to the appointment date. However, the fee cannot be refunded.
