ให้กรอกข้อมูลผ่าน ลิงก์ cmu.to/ReportCoV19 หรือ https://docs.google.com/forms/d/e/1FAIpQLSdR7-yhjpa5_--ppP1V5epCXwVdJ6f4NZa6XMt3rupUJ8pmdA/viewform
และให้สังเกตอาการตนเองอย่างใกล้ชิด
เพื่อจะได้ดำเนินการเฝ้าระวังการระบาดของโรคติดเชื้อโคโรนา 2019 (COVID-19)
และเตรียมความพร้อมในการรับมือต่อสถานการณ์ดังกล่าวต่อไป
